Three ways to register
Visit any Ministry of Labour and Social Security (MLSS) office in your parish and collect a registration form and a medical form.
Download the registration form and the medical form from the JCPD website and complete them ahead of your interview.
Visit the JCPD head office at 10 Ripon Road, Kingston 5, where the receptionist at the front desk will assist you.

Medical assessment forms
The medical eligibility report is made up of assessment forms completed by a competent medical professional. Children and adults use their own set of forms.
