Registering with the JCPD

Registration is the first step toward almost every benefit on this site. Here is how the process works, what to bring, and the medical forms involved. I can send you the registration and medical forms free of charge and walk you through each step.

Three ways to register

Where and how to begin

At an MLSS office

Visit any Ministry of Labour and Social Security (MLSS) office in your parish and collect a registration form and a medical form.

Online

Download the registration form and the medical form from the JCPD website and complete them ahead of your interview.

At the head office

Visit the JCPD head office at 10 Ripon Road, Kingston 5, where the receptionist at the front desk will assist you.

Important points to note

The interview and medical form

All applicants are required to participate in an interview.
The medical form must be completed before the interview, by a medical doctor or health-care professional competent to make the assessment.
If a parent, guardian or caregiver applies on behalf of an applicant who is able to write, the applicant must sign and date the form.
If the parent, guardian or caregiver signs on the applicant’s behalf, the form must be certified by a Senior Social Worker, JP, Principal or Medical Doctor.

Original marriage certificate (if applicable)
Government-issued ID (passport, National ID, driver’s licence, etc.)
Tax Registration Number (TRN)
Two (2) certified passport-sized photographs
National Insurance Scheme (NIS) card
